Pothole and Road Defect Report

 Reporting potholes and other defects is a simple process and it should take no more than 5 minutes, you should only fill in the parts of the form which you can - don't worry about leaving a few things blank.  After filling in the form below, click the send button to Cyclox.  A representative of Cyclox collects this information, and will forward collated reports of potholes to the appropriate person in the council.  If you are subscribed to the Cyclox yahoogroup , in which there is a lively discussion about cycling in Oxford, you will occasionally receive reports mentioning the number of potholes reported.  By submitting this form you are consenting to being contacting by email by Cyclox about the pothole you reported, if you do not wish to be contacted by Cyclox by email then do not fill in your email address.
